Cut your finger tonight chopping onions, and then do nothing about it. No ointment. No bandage. No thought.
While you sleep, your blood will seal the break. Cells on both edges will let go of their neighbors and start crawling toward each other. In two weeks there will be nothing to see. You did not supervise any of it. You were asleep.
Healing is not something we do. It is what living tissue does on its own when nothing is standing in the way.
Which turns the usual question inside out. The problem worth thinking about is not how healing happens. It is why it stops.
And for a lot of people, it does stop. The tendon that has been six weeks away from better for two years running. The gut that has been “sensitive” so long you no longer remember what it felt like before. Or nothing shows up on any test at all, and you have quietly learned to live at sixty percent and call that fine.
These are not people who have been careless with themselves. Most of them have tried harder than almost anyone I know. And their bodies still will not turn the corner. There is a specific exhaustion that comes from doing everything right and getting nowhere, and I think it is badly underestimated.
Here is what I have come to believe. In most of these cases the repair crew is not broken. The orders never arrived.
What a peptide is
Proteins are long chains of amino acids. A peptide is a short chain of the same thing. That is the only real difference.
Your body makes thousands of them, and they are not food and not vitamins. They are closer to messages. Build here. Stop the bleeding. Calm down. That is enough now. So using one as a medicine is usually not putting something strange into you and hoping. It is handing back a message your body already knows how to read, one that may have gone quiet with age, or stress, or an infection that never fully cleared.
One caution before I go on. These five are not equally proven. Thymosin alpha-1 is a prescription medicine in much of the world. BPC-157 has impressive animal research and almost no human trials. Everything below is what these molecules may do, and there is a great deal of money in this field right now, along with the overstatement that follows money.
BPC-157
This one may help if you have an injury that has stalled. Tendon, ligament, muscle, or nerve damage that has not moved in months, and problems with the gut lining, including damage from pain relievers, inflammatory bowel disease, and a gut wall that has become too leaky.
It comes from a protective substance in human stomach juice, which makes sense for an organ that sits in acid all day and rebuilds itself anyway. What it seems to do is bring blood into injured tissue. That matters most for tendons and ligaments, because almost no blood reaches them to begin with. It is the whole reason they heal so slowly. The gut is the other half, and the half I think about as a psychiatrist. When the intestinal wall gets leaky, things from inside your gut start meeting immune tissue that was never meant to see them, and years of that can look like inflammation everywhere, an immune system losing track of what belongs to you, and a low mood and brain fog that are not entirely psychiatric. Nearly all of this research was done in rats, and I am not going to pretend otherwise.
TB-500 and thymosin beta-4
This one may help when healing is slow, incomplete, or leaving scars behind. Injuries in several places at once, wounds that will not close, and situations where your body’s reaction to an injury is causing as much trouble as the injury did.
Cells crawl. That is not a metaphor. They stretch out, grab hold, and pull themselves toward a wound, and this peptide is what keeps that machinery running. Nothing gets repaired unless the right cells physically show up, and it helps them get there. It also quiets inflammation, which matters more than people realize, because much of the damage from an injury is not from the injury. It is from the response to it. The body floods the area, overshoots, and what gets left behind is scar instead of working tissue.
KPV
This one may be worth talking about if inflammation itself is the main problem rather than a side effect. Inflammatory conditions of the gut, inflammatory skin conditions, and histamine or mast cell reactions in people who have become sensitive to foods, scents, weather, and stress.
Three amino acids, snipped from the tail of a calming hormone you already make. The clever part is how it travels. Cells have doorways for pulling in small pieces of protein, and inflamed tissue builds more of them, so the peptide gets absorbed most where it is needed most. The problem draws in its own treatment. I reach for it when inflammation is the main event, and especially for histamine trouble. Histamine is the chemical behind an allergic reaction, released by immune cells that work like tripwires, and in some people those tripwires get so sensitive they start firing at things that were never a threat. Food. Weather. Perfume. A life becomes very small that way.
VIP
This one may help when the immune system has lost its off switch rather than its strength. Chronic inflammatory response syndrome, mold and biotoxin illness, illness that started with an infection and never went away, and autoimmune patterns that come from the immune system losing track of what belongs to you.
VIP is made by your nerves and also by your immune cells, which tells you those two systems are in constant conversation. What it does is adjust the immune response rather than shut it down. It makes the scouts more careful about calling something an enemy, holds back the cells that do the damage in autoimmune disease, and supports the ones whose whole job is ending a fight that is over. Mice bred so they cannot receive its message develop worse autoimmune disease than normal mice, which tells you the pathway is working quietly in all of us every day. In mold illness, and in illness that follows an infection and never resolves, the trouble is usually not that the immune system is too weak or too strong. It cannot stop. VIP is one of the few things aimed at that.
Thymosin alpha-1
This one may help if your immune system has gotten both weaker and noisier with age. Infections that keep coming back or take too long to clear, poor response to vaccines, autoimmune conditions where the goal is balance rather than more firepower, and as something studied alongside standard cancer treatment.
There is a gland behind your breastbone called the thymus, and it is where your immune cells go to school to learn the difference between you and not you. It begins shrinking when you are a teenager and is mostly fat by middle age. Almost nobody mentions this, and I think it is one of the most important facts about growing older. As it fades you get worse at answering something new precisely, and better at inflammation that has no particular target. Thymosin alpha-1 is what that gland makes, and it has more human evidence behind it than anything else here. The part I find remarkable has to do with cancer. Every cell holds up a kind of display window showing what it is making inside, so passing immune cells can check. Cancer often survives by covering the window. This peptide has been shown to make cells open it again.
What I actually think
BPC-157 and thymosin beta-4 work on repair. KPV and VIP work on the volume. Thymosin alpha-1 works on the judgment, on how well the immune system tells a real threat from a false alarm.
Most of the chronic illness I see is a breakdown in one of those three, and often all three. The person whose injury will not heal usually has something going on in the gut. The person with the gut problem usually has histamine symptoms. The person with histamine symptoms often has autoimmune disease in the family. These are not different patients. That is one patient, described from different angles by specialists who are not talking to each other.
Hope is not a small thing to hand someone, and I try not to be careless with it. Peptides are not magic. They do not replace sleep, or protein, or treating the infection that started all this. They need a doctor who knows your history and an honest plan for how you will know whether it worked. But if the basics are handled and your body still will not heal, the idea that a signal has gone quiet deserves to be taken seriously.
One more thing. The body is not a machine, and healing cannot be talked about as though it were. Someone who has been unwell for years is not only carrying inflammation. They are carrying grief, and fear, and a great deal of not being believed.
Sometimes the repair crew is standing right there, fully staffed, and never got the message. These molecules may be a way to send it again. The rest of the work, the part about living and moving toward life, is still yours. It is worth doing.
