Dear readers,

Somewhere around age twenty-five, without any announcement, your skin quietly changed jobs. It stopped building and started maintaining. And maintenance runs at a deficit.

From that point you lose roughly one percent of your collagen every year. For women moving through menopause the drop is much steeper, as much as thirty percent in the first five years alone. Elastin, the protein that lets skin snap back, is worse off still. Your body makes very little new elastin after adolescence. What you have is largely what you get.

You do not feel any of this happening. That is the whole problem with it. There is no day when it hurts. There is just a photograph from four years ago that you find yourself looking at a little longer than you meant to.

Here is the good news, and it is the reason I am writing this in September.

Fall is when we can actually do the work.

Why now

Almost everything effective we do to skin makes it temporarily more vulnerable to the sun. Resurfacing removes a layer. Heat and needling send the skin into an active repair state. During that window, ultraviolet light is not a minor annoyance. It is the thing most likely to undo your result and leave you with uneven pigment.

Which is why summer is not the best season for this and why the months in front of us are the right one. Shorter days. Fewer beach weekends. Long sleeves and hats that you were going to wear anyway.

There is a second reason, and it has to do with time. Collagen does not appear the week after a treatment. The remodeling happens over roughly three months, quietly, underneath. If you start now, your skin is finishing the work through the fall and arriving somewhere genuinely different by the time the holidays come and the new year starts.

Start in April and you are asking your skin to rebuild itself in the middle of the sunniest stretch of the year. It is simply a harder ask.

The approach I have taken

I want to say something about philosophy before I talk about machines, because the philosophy is what actually determines whether you like your face in ten years.

I have taken the approach of trying to preserve my collagen and elastin rather than trying to get it back once it is gone.

This is not a small distinction. It is the entire difference between two very different experiences of aging.

Rebuilding lost structure is possible, and we are better at it than we have ever been. But it is harder, it takes more sessions, it costs more, and it never fully returns what was there. Holding onto what you already have is cheaper, gentler, and far more effective. The best time to start treating your skin is a few years before you think you need to.

I understand that runs against how most people approach this. Most people wait until something bothers them enough to act, which by definition means waiting until the loss is visible. Visible loss is late loss. By the time you can see laxity along the jaw, the underlying scaffolding has been thinning for a decade.

So if you are in your thirties and reading this thinking you are too early, you are not early. You are on time.

And if you are past that point and the changes are already there, do not read that as bad news. It only means we start from where you are, with a slightly different plan and a bit more patience. Nothing is lost.

What actually bothers people, and what to do about it

When people sit down and tell me what they see in the mirror, it tends to sort into a few categories.

Texture, tone, and sun damage. The skin looks a little dull. Pores read larger than they used to. There is a scattering of brown from summers you barely remember. Fine lines have started to show up around the eyes and across the upper lip even when your face is at rest. This is surface and upper dermal damage, and it is largely a resurfacing problem.

Laxity. The jawline is softer than it was. There is fullness under the chin that has nothing to do with your weight. Cheeks sit a little lower. Skin on the neck has begun to look crepey. This is structural, and it lives deeper than any peel or topical can reach. This is a remodeling problem.

Scarring. Acne scars from fifteen or twenty-five years ago that you have made peace with but would still rather not have.

Everything at once. Which, honestly, is most people over forty.

The reason we use two different technologies is that these are genuinely different problems living at different depths. One tool does not do both jobs well.

CoolPeel

CoolPeel is a fractional CO2 laser treatment, and the clever part of it is in the timing.

Traditional CO2 resurfacing works beautifully, and it is also why people spent decades associating lasers with a week of hiding. The laser removes damaged tissue, but heat spreads outward into the surrounding skin, and that spread is what produces the long red recovery.

CoolPeel delivers energy in extremely short, high-power pulses. The targeted tissue is vaporized and gone before meaningful heat has time to travel sideways into the skin around it. You get the resurfacing without the collateral thermal injury.

What that means practically is that we can treat fine lines, uneven tone, sun damage, pore size, texture, and superficial scarring, and you will typically look pink and feel a bit like sandpaper for a few days rather than being out of commission for two weeks. Most people are comfortable in makeup and back in the world quickly.

It is also a treatment that rewards a series. One session does real work. Three sessions spaced out over the season do considerably more.

This is the tool for the top layers. Clarity, brightness, smoothness, the quality of the skin itself.

Morpheus8

Morpheus8 is a different animal entirely, and it goes where the laser cannot.

It combines microneedling with radiofrequency energy. Fine needles pass through the surface and deliver controlled heat directly into the dermis and, at deeper settings, into the subdermal fat. Depths are adjustable from half a millimeter down to eight, which means we can treat delicate skin around the eyes and dense tissue under the jaw with the same platform and completely different parameters.

What that heat does is trigger a remodeling response. The tissue contracts, and then over the following weeks fibroblasts lay down new collagen and elastin. In studies looking at treated tissue under the microscope, collagen increased by roughly a quarter and elastin by about a third. A retrospective review of nearly 250 patients found meaningful improvement in laxity of the lower face and neck, with satisfaction above ninety percent.

This is the treatment for jawline definition, submental fullness, crepey neck skin, and acne scarring. It also works well on the body, on abdomen, arms, and knees.

One more thing worth knowing. Because radiofrequency energy is not absorbed by pigment the way laser light is, Morpheus8 is considerably safer across the full range of skin tones. If you have deeper skin and have been told that lasers are risky for you, this is very often the answer.

Results build. You will notice something in a few weeks and the real result at three months. Most people do a series.

Putting it together

Used well, these two are complementary rather than competing. Morpheus8 rebuilds and tightens the architecture underneath. CoolPeel refines the surface sitting on top of it. Structure and finish.

A realistic year looks something like this. We begin in the fall with a series, spaced out over several weeks, chosen for whatever your primary concern actually is. Your skin does its remodeling work through the winter. By spring, you are in maintenance rather than correction, which usually means far less frequent treatment, excellent daily sun protection, and a home routine that is actually supporting the investment you just made.

That is the whole strategy. Do the meaningful work in the dark months. Protect it in the bright ones. Repeat.

Maintenance is not a lesser version of treatment. Maintenance is the point.

One last thing

I think it matters to say plainly that wanting to look like yourself, rested and well, is not vanity and it does not require an apology.

Every woman deserves to feel good in her body.

I have watched what happens when someone stops avoiding mirrors. It is not really about the mirror. It shows up in how she carries herself in a room, in photographs she is willing to be in, in a kind of ease that had gone missing and comes back. That is not a small thing to give someone, and I have never once thought it was frivolous.

The goal was never a different face. The goal is your own face, holding onto what it has for as long as it possibly can.
