FDA first convened a Joint Meeting of the Bone, Reproductive and Urologic Drugs and the Drug Safety and Risk Management Advisory Committee in September 2014 following increased reports of stroke, heart attack, and death in men taking FDA-approved testosterone products.

By 2015, the FDA required all testosterone product manufacturers to add a black box warning about possible increased risk of heart attacks and strokes in patients taking testosterone. This warning was based on limited, often flawed observational studies that showed conflicting results. The decision created a chilling effect on testosterone prescribing, leaving millions of men with legitimate medical needs without access to treatment.

The TRAVERSE Trial: Science Over Fear

The FDA’s recent decision is based primarily on the results of the TRAVERSE (Testosterone Replacement Therapy for Assessment of Long-term Vascular Events and Efficacy Response in Hypogonadal Men) trial, a multicenter, randomized, double-blind, placebo-controlled, noninferiority, event-driven trial that was specifically mandated by the FDA to address cardiovascular safety concerns.

Study Design and Participants

The TRAVERSE trial was impressive in both scope and rigor. We enrolled 5246 men 45 to 80 years of age who had preexisting or a high risk of cardiovascular disease and documented hypogonadism. These weren’t healthy young men looking for a performance boost – these were men with genuine medical conditions who could benefit from testosterone therapy.

Importantly, all participants also had to have two fasting serum testosterone results below 300 ng/dL from morning blood draws separated by at least 48 hours. “These are men with documented mild to moderate hypogonadism,” Dr. Lincoff notes.

The study followed men for a mean 27.1 months, with participants receiving either testosterone gel or placebo, carefully titrated to maintain testosterone levels in the normal range.

The Results That Changed Everything

The primary finding was clear and decisive: In men with hypogonadism and preexisting or a high risk of cardiovascular disease, testosterone-replacement therapy was noninferior to placebo with respect to the incidence of major adverse cardiac events.

To put this in perspective, the 372 confirmed cardiovascular events observed in this trial were more than all prior randomized trials of testosterone combined. “This is the most definitive answer to this research question we are likely to get,” noted the study’s lead investigator.

The evidence now suggests that rather than causing cardiovascular problems, low testosterone itself may be the real risk factor. Men with low testosterone have higher rates of diabetes, cardiovascular disease, depression, and overall mortality.
